Job summary

PurposeCare is seeking a Licensed Practical Nurse to join our home health team in Illinois. You will deliver skilled nursing visits in patients' homes, assess needs, and coordinate care with caregivers and physicians.

The role offers flexible scheduling, autonomy, and opportunities for professional growth through training and referrals to other PurposeCare locations. A valid LPN license and 1 year of nursing experience required.

Qualifications

• Current license with the state Board of Nursing.
• 1 year of nursing experience required; home health and OASIS experience preferred.
• Ability to perform tasks involving physical activity, including lifting up to 50 lbs.
• Valid Driver's License and access to reliable transportation.
• Successful background check.
