Job summary

Interim HealthCare of Oklahoma City is seeking Home Health LPNs (full-time and part-time) to provide skilled nursing visits in patients’ homes across the OKC metro. You’ll work with an RN case manager, influence how your day runs, and join a locally owned agency with a community focus.

Full-time and part-time shifts are available with weekend requirements; visits are scheduled near where you live when possible and may extend across the metro based on patient needs.

Qualifications

• Active Oklahoma LPN license (or multistate compact) is required.
• At least 1 year of nursing experience; home health is a plus.
• Ability to work independently in patients' homes.
• Strong communication and thorough documentation.
• Valid driver’s license, reliable transportation and auto insurance.
• Current CPR certification.

Responsibilities

• Provide skilled nursing visits in patients' homes per plan of care.
• Perform assessments, vital signs, wound care, medications and education.
• Educate patients and families about diagnosis, meds and home safety.
• Report changes to RN case manager and physician promptly.
• Document each visit accurately in the EMR.
• Coordinate with RNs, therapists, aides and office team to stay on track.

Job description

Are you an LPN who wants more time with each patient and more say in how your day runs? Interim HealthCare of Oklahoma City is hiring Home Health LPNs, full-time and part-time, to provide skilled nursing visits in patients’ homes across the OKC metro. We’re especially looking for nurses in Northwest Oklahoma City, Bethany, Warr Acres, The Village, and Edmond, but we hire LPNs wherever they live in the metro. As a Home Health LPN, you’ll build real relationships with your patients, work under a plan of care with an RN case manager behind you, and have a local team that picks up the phone when you call.

Schedule

• Full-time Home Health LPN: two weekends per month, with a weekday off following each weekend worked
• Part-time Home Health LPN: one weekend per month
• Weekday visits are scheduled with your input, and we work to keep visits near where you live when patient needs allow
• Staffing and patient needs can mean visits outside your area, so flexibility across the metro is important

What you’ll earn

• Full-time Home Health LPN: $30 per visit
• Part-time Home Health LPN: $22 per visit
• On-call: $40 per day Monday through Friday | $100 per day Saturday and Sunday
• Mileage reimbursement: $0.64 per mile
• Weekly pay every Friday

What you’ll do

• Provide skilled nursing visits to home health patients according to the plan of care established by the RN case manager
• Perform assessments, vital signs, wound care, and medication management and education
• Teach patients and families about their diagnosis, medications, and home safety
• Report changes in condition to the RN case manager and physician in a timely way
• Document each visit accurately and on time in our electronic medical record
• Coordinate with RNs, therapists, aides, and the office team to keep care on track

What we’re looking for

• An active Oklahoma LPN license or multistate compact license
• 1 year of nursing experience; home health experience is a plus
• Confidence working independently in the home setting
• Strong communication and documentation skills
• A valid driver’s license, reliable transportation, and current auto insurance
• Current CPR certification
