Job summary

Liberty Home Care seeks an experienced HOME HEALTH NURSE – LPN to provide direct patient care in home settings, following physician orders and the plan of care. You will coordinate with RNs, Patient Care Coordinator and other staff to ensure safe, quality home health services.

Responsibilities include completing documentation, attending care conferences, and communicating effectively with patients and families.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located.
• One year of clinical nursing experience, preferably in an acute care setting.
• Ability to independently organize work and provide effective home health care.
• Effective communication skills (oral and written).
• CPR certification.

HOME HEALTH NURSE – LPN

Job Description:

• Provides direct patient care, to include using a variety of skills and necessary coordination of services.
• Completes appropriate documentation as required by policy and procedure and regulation.
• Follows the plan of care appropriate to the patient’s diagnosis and condition as developed and assigned by the Registered Nurse.
• Coordinates homecare services with the Registered Nurse, Patient Care Coordinator and other agency personnel to optimize patient care, as well as, the patient, family, community agencies, and physicians.
• Accepts and transcribes physician’s orders as directed.
• Forwards all clinical, progress, and summary notes to the Registered Nurse for coordination.
• Attends and participates in patient care conferences.

Job Requirements:

• Successful completion of nursing education from an approved school of nursing.
• Current LPN licensure in the state in which the branch is located and one year of clinical nursing experience, preferably in an acute care medical/surgical setting.
• Ability to independently organize work and provide effective health care services to homebound patients.
• Ability to ensure appropriate patient care as supervised by the home health nurse – Registered Nurse, and document thoroughly, accurately, and completely on a timely basis.
• Effective communication skills (oral and written).
• CPR certification.
