Job summary

Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to support patients in staying at home, providing compassionate care and education to families. This role emphasizes patient-centered goals, safety, and quality of life, with a focus on empowering patients to manage their health at home.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state license as a Licensed Practical Nurse.
• Current driver’s license.
• Current CPR certification.
• Negative TB screen and hepatitis consent/declination.
• Two years of experience as an LPN in a clinical care setting, home health preferred.

Responsibilities

• Support patient recovery and ability to stay at home - enabling them to spend quality time with loved ones.
• Educate patients and families to reach healthcare goals and disease management.
• Assist with care coordination and planning in home health settings.
