Job summary

Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to support patient recovery at home and help families achieve healthcare goals. The role emphasizes compassionate, home-based care with a focus on education and outcomes.

Qualifications

• Graduate of an accredited Practical Nursing program with an active LPN license.
• Current driver’s license and CPR certification.
• Two years of LPN experience in a clinical care setting; home health preferred.
• Negative TB screen and hepatitis consent/declination completed.

Responsibilities

• Support patient recovery and enable at-home care so they can spend time with loved ones.
• Educate patients and families to help reach healthcare goals and disease management.
