Sheridan Memorial Hospital is seeking a Home Care and Hospice Registered Nurse to manage an assigned caseload in home health and hospice settings in Sheridan, WY. This full-time role operates days, Mon-Fri 8:00 AM–4:30 PM with occasional on-call requirements.

The RN will plan, coordinate, provide, and document patient care in alignment with regulatory standards and agency policies. The ideal candidate will hold a Wyoming RN license, have an associated degree in nursing (Bachelor’s preferred),

Competenze

• Associated degree in nursing is required.
• Bachelor’s degree in Nursing is preferred.
• Wyoming RN license without restrictions.
• Current BLS certification.
• Central Registry background checks required.

Mansioni

• Develops and documents care plans for home hospice patients.
• Provides skilled nursing and pain management.
• Collaborates with hospice IDG and physicians to coordinate care.
• Communicates patient status and updates to family and team.
• Provides on-call coverage as needed.
