Job Description

The Registered Nurse coordinates and directs the home care patients care based on individual patient needs. The RN independently manages the Home Health/Hospice patient population using advanced assessment, teaching, and decision-making skills. The nurse ensures appropriate referrals to other services, facilitates interdisciplinary conferencing, and completes required documentation. Relevant knowledge and experience are applied to new patient populations.

Our Home Health Care Mission is to provide professional and paraprofessional services to patients in their homes, assisting them to achieve the highest level of potential in their day-to-day activities.

Responsibilities and Duties

The Home Health Primary Nurse must possess excellent communication skills, both written and verbal.
Provide comprehensive assessment, planning, implementation, and evaluation for a caseload of home care clients as the primary nurse.
Maintain a complete record for the care given with the patient, documenting in the clinical record per agency policy and procedure.
Keep an updated clinical record on each patient, meeting deadlines for documentation, supervision reports, care plan updates, progress toward goals, and verbal orders.
Accurately complete admission and revisit documentation, communications, billing, employee statistics, and payroll documentation.
Document patient, family and caregiver teaching.

Patient Care

Complete initial assessments of patient and family to determine home care needs; provide a complete physical assessment and history of illnesses.
Regularly re-evaluate patient nursing needs and revise the plan of care as patient status changes.
Identify discharge planning needs as part of the care plan and implement prior to discharge.
Act as Case Manager when assigned and coordinate patient care for the assigned caseload.

Additional Duties

Participate in on-call duties as defined by policy.
Ensure arrangements for equipment and other necessary items and services are available.
Instruct, supervise, and evaluate home health aide care provided every two weeks.

Licenses and Experience

Current RN License in the State of California
Minimum 1 year of recent professional nursing experience, in home health, hospice, or public health
Current CPR certification
