Qualifications

• Graduate of an approved professional nursing program.
• Active California RN license in good standing.
• Minimum of 1 year of professional nursing experience.

• OASIS experience preferred.

• Valid California driver’s license.
• Current auto insurance and reliable transportation.
• Ability to pass criminal background check, DMV check, and onboarding requirements.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide skilled nursing services within the scope of nursing practice.
• Complete comprehensive patient assessments, including medical history and home safety review.
• Perform assessment visits and timely documentation (OASIS and nursing notes).
• Develop, implement, and revise Plans of Care with physician and team.
• Identify needs and determine if additional home health services are needed.
• Re-evaluate nursing needs and ADL/iADL abilities.
• Supervise Home Health Aide services per state/federal requirements.
• Educate patients and families on disease management and self-care.
• Communicate status changes to physicians and care team.
• Coordinate patient services with care team and payers.
• Participate in case conferencing and quality improvement.
• Review billing concerns with patients/families when applicable.
• Maintain licensure and comply with regulations.

Job Summary

We are seeking a compassionate, dependable, and clinically skilled Per Diem Home Health Registered Nurse to join our growing home health team.

The Home Health RN provides professional skilled nursing services within the scope of nursing practice standards and under the supervision of the Director of Patient Care Services. The RN works closely with physicians, patients, families, and the interdisciplinary care team to provide high-quality, patient-centered care in the home.

Key Responsibilities

• Provide skilled nursing services requiring substantial and specialized nursing knowledge.
• Complete comprehensive patient assessments, including medical history, physical assessment, home safety review, and environmental assessment.
• Perform assessment visits and complete timely documentation, including OASIS assessments and skilled nursing notes as required.
• Develop, implement, and revise individualized Plans of Care in collaboration with the physician and interdisciplinary team.
• Identify patient needs and determine when additional home health services may be medically necessary.
• Regularly re-evaluate the patient’s nursing needs, functional status, ADL and iADL abilities, and need for home health aide services.
• Develop, revise, and sign the Home Health Aide plan of care when HHA services are ordered.
• Supervise Home Health Aide services in accordance with state and federal requirements and document supervision timely.
• Provide patient and family education regarding disease management, medications, self-care techniques, safety, and the plan of care.
• Communicate patient status changes, needs, and concerns to the physician, Director of Patient Care Services, Nursing Supervisor, and other disciplines as appropriate.
• Coordinate patient services with nursing, therapy, social work, home health aide services, physicians, payers, and community resources.
• Participate in case conferencing to support best practices and quality patient outcomes.
• Submit timely requests for re-authorization or additional visits when more visits are needed.
• Review billing-related concerns with the patient and/or family when applicable, including situations where co-pay or Medicare coverage may not apply.
• Participate in clinical record reviews, performance improvement activities, staff meetings, and in-services as needed.
• Support the growth of the home health division by maintaining professional communication, strong documentation, and a team-centered approach.
• Maintain professional licensure and comply with agency policies, Medicare Conditions of Participation, and applicable state and federal regulations.
• Notify the agency promptly of emergencies, illness, schedule changes, or other issues that may impact patient care or caseload coverage.
• Perform other duties as assigned.

Flexible schedule based on patient needs and clinician availability.

Compensation

Per diem / per-visit compensation.

Estimated pay range: $95-$165 per visit depending on visit type, documentation requirements, and clinical complexity.

Visit types may include routine skilled nursing visits, Start of Care, Resumption of Care, Re-certification, Discharge OASIS, supervisory visits, clinical record review, and approved administrative time.

Work Environment

This position requires travel to patient residences throughout the assigned service territory. Patient care is performed in the patient’s home, which may include exposure to communicable diseases and home environments that may not be handicap accessible.

Physical Requirements

• Ability to walk, stand, sit, reach, bend, stoop, climb stairs, and use a telephone, computer, and other office or clinical equipment.
• Ability to access patient homes and complete routine field visits.
• Ability to assist with patient care needs and use transfer or lifting devices when appropriate.
• Must have reliable transportation to complete routine patient visits.
