Job summary

BayCare HomeCare is hiring a Home Health RN in the New Port Richey, FL area. The role involves hands-on nursing in patients' homes, coordinating a multidisciplinary team, ensuring visits align with physician orders, and maintaining high-quality care and patient satisfaction.

Requirements include BLS, an active RN license in Florida, and a valid driver's license. Prior home health experience and strong communication skills are preferred; occasional weekends and on-call duties apply.

Qualifications

• BLS certification required.
• RN license in good standing.
• Driver's license required within 30 days of hire.
• 1 year RN experience in home health preferred.
• Strong communication and compassionate patient interaction.

Responsibilities

• Oversee activities of the multidisciplinary care team.
• Monitor therapeutic services via written/ verbal communication.
• Ensure visits are authorized and covered by physician orders.
• Review weekly visits and provide care plan updates to supervisor.
• Inform scheduler of changes and communicate caseload issues.

• Facility: BayCare HomeCare
• Location: New Port Richey, FL
• Status: Full-Time
• Shift Hours: 8:00 AM – 5:00 PM, Monday - Friday
• Weekend Work: Every 3rd
• On Call: Yes
• Home visits in assigned service area: Pasco County

The Home Health Field RN provides hands‑on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Home Health RN Requirements:

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Required Associate's or Diploma Nursing
• Required 1 year Registered Nurse
• Preferred Bachelor's Nursing
• Preferred 1 year Home Care
