Job summary

BayCare HomeCare is seeking an experienced Home Health Field RN in Largo, FL. The role offers a flexible schedule with occasional weekend work and on-call responsibilities, delivering hands-on in-home nursing to patients within the assigned service area.

The successful candidate will have an RN license, BLS, and a Florida driver’s license (within 30 days of hire) with 1 year of RN experience; an Associate’s or Diploma in Nursing is required, and a Bachelor’s is preferred.

Responsibilities

• Oversee the activities of the multi-disciplinary team that provides care to the patient.
• Monitor therapeutic services through written and verbal communication with all disciplines.
• Ensure visits are authorized and covered by physician orders.
• Review patient visits weekly and communicate with the Patient Care Supervisor and team.
• Inform scheduler of changes and communicate caseload and issues.

• Shift Hours: 8:00 AM - 5:00 PM
• Weekend Work: Every Other

• Home visits in assigned service area: Largo, St. Petersburg, Pinellas Park surrounding areas assigned by zip code.

Responsibilities

The Home Health Field, RN provides hands‑on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Home Health RN Requirements

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Required Associate's or Diploma Nursing
• Required 1 year Registered Nurse
• Preferred Bachelor's Nursing
• Preferred 1 year Home Care
