Job summary

BayCare HomeCare in Tampa is seeking a Home Health RN who provides patient-centered care in the home setting. The role emphasizes coordination with a multidisciplinary team and adherence to nursing protocols within the BayCare network.

The ideal candidate has at least 1 year of RN experience, an active Florida RN license, and BLS certification. Working hours are 8:00 AM - 5:00 PM, Monday to Friday, with on-call duties.

Qualifications

• BLS certification.
• Driver's license (Florida) required within 30 days of hire.

Responsibilities

• Oversee the activities of the multi-disciplinary team that provides care to the patient.
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines.
• Ensure that all visits scheduled and provided are authorized and covered by physician orders.
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits.
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

The Home Health RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Oversee the activities of the multi-disciplinary team that provides care to the patient

• Ensure that all visits scheduled and provided are authorized and covered by physician orders

Required Certifications and Licensures:

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Education:

• Required Associate's or Diploma Nursing
• Preferred Bachelor's Nursing

Experience:

• Required 1 year Registered Nurse
• Preferred 1 year Home Care
