Job summary

BayCare HomeCare Lakeland is seeking a dedicated Home Health Field, RN to deliver hands-on nursing care in patients' homes within Lakeland, FL. You will lead a multidisciplinary team, ensure visits follow physician orders, and uphold clinical standards in every interaction.

The role emphasizes coordination, communication, and scheduling accuracy, with opportunities for mentorship, training, and career growth within BayCare.

Qualifications

• BLS (Basic Life Support) certification is required.
• RN license is required.
• Florida driver’s license within 30 days of hire is required.
• Associate's or Diploma in Nursing is required.
• Minimum 1 year as a Registered Nurse.

Responsibilities

• Oversee the activities of the multidisciplinary team providing in-home care.
• Monitor therapeutic services and coordinate with all disciplines.
• Ensure visits are authorized and aligned with physician orders.
• Review patient visits weekly and communicate with the supervisor and team.
• Inform scheduler of schedule changes and caseload issues.

• Shift Hours: 8:00 AM - 5:00 PM
• Weekend Work: Every 3rd
• On Call: No
• Home visits in assigned service area: Lakeland, FL

The Home Health Field, RN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review of patient visits weekly and providing plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor

Home Health RN Requirements:

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License State of Florida (Florida driver's license is required within 30 days of hire)
• Required Associate's or Diploma Nursing
• Required 1 year Registered Nurse
• Preferred Bachelor's Nursing
• Preferred 1 year Home Care
