Job summary

BayCare HomeCare is seeking a HomeCare Physical Therapist to evaluate and develop individualized home health treatment plans. The role involves providing clinical direction to PT Assistants and coordinating with a multidisciplinary team to ensure quality care in patients' homes. Requirements include a Bachelor’s Degree in Physical Therapy, Florida PT license, and 1 year of PT experience.

A valid Florida driver’s license and BLS are also required; Master’s degree is preferred. 8:00 a.m.

Qualifications

Responsibilities

• Evaluates and develops therapy programs for home health patients.
• Provide clinical direction for PT Assistants.
• Plan, implement, manage, and evaluate home health services to meet patient needs.
• Coordinate with multi-disciplinary team to deliver quality care.
• Communicate plan and discharge plan with patient/family and health team.
• Adjust schedules and caseload in collaboration with supervisor.

• Home visits in assigned service area: Pasco County.

HomeCare Physical Therapist - Responsibilities include :

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.

• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.

• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.

• Manages the activities of the multi-disciplinary team providing care to the patient.

• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.

• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.

• Interprets agency policy to patients/families.

• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.

• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

HomeCare Physical Therapist Requirements:

• Bachelor's Degree Physical Therapy

• Physical Therapist State of Florida License

• 1 year Physical Therapist experience

• BLS (Basic Life Support)

• Driver's License (Florida driver's license is required within 30 days of hire)

• Preferred Master's Degree Physical Therapy
