Job summary

BayCare HomeCare is seeking a Home Health Physical Therapist to evaluate patients, develop individualized treatment plans, and provide top-quality home-based care across designated service areas in Dunedin and surrounding communities in Florida.

The role requires a Florida Physical Therapy license and at least 1 year of experience in physical therapy, with a preference for candidates who hold a Master's in Physical Therapy. A valid Florida driver’s license and BLS certification are also required.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Florida Physical Therapy license required.
• 1 year Physical Therapist experience required.
• Basic Life Support (BLS) certification required.
• Driver's License (Florida) required within 30 days of hire.
• Master's Degree in Physical Therapy preferred.

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Provides clinical direction for Physical Therapy Assistants as needed.
• Plans, implements, manages and evaluates home health services to ensure patient needs are met and quality care is provided.
• Manages activities of the multi-disciplinary team providing care to the patient.
• Monitors therapeutic services and communicates with all disciplines.
• Develops and carries out a discharge plan with patient, family/caregiver and health care team.

• Location: Mease Dunedin Physician Office Bldg. and HomeCare – Dunedin, FL
• Status: Full-Time, Exempt
• Shift Hours: 8:00 a.m. - 5:00 p.m., Monday-Friday
• Weekend Work: Every 3rd
• On Call: No
• Home visits in assigned service area: Dunedin, Palm Harbor, Tarpon Springs, Oldsmar, and surrounding areas - surrounding areas assigned by zip code.

HomeCare Physical Therapist - Responsibilities include :

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

HomeCare Physical Therapist Requirements:

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1-year Physical Therapist experience
• BLS (Basic Life Support)
• Driver's License (Florida driver's license is required within 30 days of hire)
• Preferred Master's Degree Physical Therapy
