Job summary

BayCare HomeCare in Largo, FL is seeking a dedicated Physical Therapist Educator for a PRN, per-visit role in home health. The candidate will begin with 5–6 weeks of full-time Home Health training before taking on educator responsibilities and patient visits.

The position offers flexible scheduling, opportunities for growth, and the chance to impact patients in their homes across Largo and surrounding areas.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Florida PT license required.
• 1-year PT experience preferred.
• Driver's License required within 30 days of hire.
• Background screening in AHCA system is required.

Responsibilities

• Evaluates and develops PT treatment programs for home health patients.
• Provides clinical direction for Physical Therapy Assistants.
• Plans and manages delivery of home health services and case management.
• Collaborates with multidisciplinary team to meet patient needs.
• Develops discharge plans with patient and family.

• Shift Hours: 8:00 AM - 5:00 PM

• Home visits in assigned service area: Largo, St Petersburg, Pinellas Park, Seminole, and surrounding areas.

Responsibilities

• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
• Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
• Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients need are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
• Manages the activities of the multi-disciplinary team providing care to the patient.
• Monitors the provision of therapeutic services through written and verbal communication with all disciplines.
• Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
• Interprets agency policy to patients/families.
• Reviews patients visit weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
• Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

Home Health Physical Therapist Requirements

• Bachelor's Degree Physical Therapy
• Physical Therapist State of Florida License
• 1-year Physical Therapist experienceBasic Life Support
• Driver's License (Florida driver's license is required within 30 days of hire)
• Preferred Master's Degree Physical Therapy
