Job summary

Sunrise Home Health is seeking a Certified Occupational Therapy Assistant (COTA) to join our therapy team in the home health setting. You will work with Occupational Therapists to deliver individualized therapy focused on improving daily function, safety, and independence in patients’ homes.

You will conduct evaluations, develop and adjust treatment plans, and collaborate with families and physicians to ensure coordinated, high-quality care in a flexible home-based environment.

Qualifications

• Education and Experience: Must be a graduate from an accredited Occupational Therapy program and licensed by the state of Arizona.

Responsibilities

• Assist in evaluating patient function and safety in the home setting.
• Assess home environment for safety hazards and determine equipment needs.
• Identify functional problems and establish goals and plan of care to improve function.
• Perform tests and measures (ROM, manual muscle tests, gait) and document findings.
• Complete start-of-care and ongoing assessments per agency policy.
• Re-evaluate and adjust patient programs to maximize improvement.
• Administer prescribed therapy programs and therapeutic exercises.
• Recommend/adjust prosthetic or orthotic devices as needed.
• Communicate with family, physician, and care team for coordinated care.
• Submit evaluation, plan of care, notes and progress reports within 48 hours.

We are seeking a motivated and compassionate Certified Occupational Therapy Assistant (COTA) to join our growing therapy team.

As a COTA with Sunrise Home Health, you’ll work closely with our Occupational Therapists and interdisciplinary care team to provide individualized, patient-centered therapy focused on improving daily function, safety, independence, and overall quality of life. Key Deliverables

• Assists the physician in evaluating the patient's level of function.
• Assess patient's home environment for safety hazards and architectural barriers identify equipment needs.
• Identifies functional problems and establishes goals and a plan of care designed to improve the patient's functional status (revising as necessary).
• Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
• Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance to agency policy.
• Periodically re-evaluates and upgrades the patient's program to maximize functional improvement.
• Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
• Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
• Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
• Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
• Performs and submits required documentation of patient's evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
• All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48) hours.
