Job summary

Sunrise Home Health in Mesa, AZ is seeking a Certified Occupational Therapy Assistant (COTA) to join our therapy team serving the Maricopa, Casa Grande, Coolidge, Florence and surrounding areas.

You will work closely with Occupational Therapists to provide individualized, patient-centered therapy focused on improving daily function, safety, independence, and overall quality of life in patients’ homes. The position offers a flexible schedule with part time or full time options.

Qualifications

• Graduate from an accredited Occupational Therapy program and licensed in Arizona.

Responsibilities

• Assist the physician in evaluating the patient’s level of function.
• Assess home environment for safety hazards and architectural barriers; identify equipment needs.
• Identify functional problems and establish goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
• Perform patient tests, measurements, and evaluations (ROM, manual muscle tests, gait analysis, body parts measurement) and record findings.
• Complete start-of-care and ongoing assessments and associated paperwork per agency policy.
• Periodically re-evaluate and upgrade the patient’s program to maximize functional improvement.
• Administer the exercise program using specialized therapeutic techniques and equipment.
• Plan and administer prescribed therapy programs to restore function and prevent disability.
• Recommend prosthetic/orthotic devices or adjust devices as needed with prosthetist/orthotist.
• Coordinate with family, referring physician, and care team to promote coordinated care.
• Submit evaluation, plan of care, clinical notes, and progress reports within 48 hours.

Key Deliverables:

• Assists the physician in evaluating the patient’s level of function.
• Assess patient’s home environment for safety hazards and architectural barriers identify equipment needs.
• Identifies functional problems and establishes goals and a plan of care designed to improve the patient’s functional status (revising as necessary).
• Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
• Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance to agency policy.
• Periodically re-evaluates and upgrades the patient’s program to maximize functional improvement.
• Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
• Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
• Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
• Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
• Performs and submits required documentation of patient’s evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
• All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48) hours.

Qualifications: Education and Experience:

• Must be a graduate from an accredited Occupational Therapy program and licensed by the state of Arizona.
