Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Valid LICSW license in good standing.
• Minimum 1 year health care experience; home health preferred.
• Excellent verbal and written communication; strong organizational skills.
• Maintain current BLS CPR certification.
• Valid driver's license and own reliable vehicle.

Responsibilities

• Assess patients' psychosocial status in relation to illness and environment.
• Share assessment findings with the home health team for holistic care.
• Develop intervention plans based on assessments to improve care outcomes.
• Maintain accurate clinical records for referred patients.
• Provide information and referrals to patients, families, and caregivers.
• Liaise with patients/families and community agencies to access resources.
• Collaborate with the interdisciplinary team to develop plans of care and participate in case conferences.
• Assist physicians and team members in understanding social and emotional factors impacting health.
• Participate in discharge planning to ensure appropriate post-care support.
