Job summary

AuthoraCare Collective in High Point, NC, is seeking a Home Health Social Worker to provide psychosocial support to patients and families, helping them navigate illness, disability, and recovery in a home setting. This position offers a flexible 24-hour-per-week schedule and office-based base, with the opportunity to impact care within an interdisciplinary team.

The MSW assesses social and emotional needs, develops individualized plans, connects clients with community resources, and advocates

Qualifications

• Master’s degree in social work required.
• One year of social work experience in a mental health or medical setting is required.
• Must be able to travel to, enter, and function in the homes of patients within the service area.
• Valid state-issued driver’s license and own transportation required.

AuthoraCare Collective is currently seeking a Home Health Social Worker supporting various service areas. Office base - High Point. This is a part-time position (24 hours per week) with a flexible schedule.

Job Summary

The Social Worker in a home health setting provides psychosocial support and counseling to patients and their families, helping them navigate the emotional, social, and environmental challenges associated with illness, disability, and recovery.

Working as part of an interdisciplinary care team, the MSW assesses patients’ social and emotional needs, develops individualized care plans, and connects patients with community resources to promote optimal health outcomes and independence at home.

Requirements

Education/Qualification

• Master’s degree in social work
• One year of social work experience in a mental health or medical setting is required
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation

Key Responsibilities

Delivery of Care

• Completes comprehensive psychosocial assessments, providing counseling and crisis intervention, assisting with advance care planning, and connecting patients to community resources including but not limited to financial, housing and transportation support
• Works in collaboration with the interdisciplinary team and home health director
• Develops and carries out individualized plans of care related to the social determinants of health
• Identifies appropriate resources to meet patient/caregiver/family needs
• Serves as an advocate for patients and families

Interdisciplinary Team Process

• Actively participates with the interdisciplinary group (IDT)
• Works effectively with other IDT members with a focus on collaboration in problem-solving patient needs and working with the patients’ plan of care
• Provides effective report to the IDT based on regulatory requirements for patient care planning evaluation
• Maintains effective relationships and partnerships with community healthcare providers, agencies, and senior services

Documentation

• Completes required documentation according to agency accreditation standards
• Ensures that notes and patient records are complete, accurate, current and clear
• Documents consultation and/or referrals to Home Health leaders for clinically complex cases requiring advanced clinical interventions
