Job summary

Luxe Home Health LLC seeks an experienced Clinical Team Manager (RN) to lead in-office clinical operations for home health patients and clinicians. You will coordinate care, provide clinical oversight, and ensure compliant, high-quality patient care across the interdisciplinary team.

Responsibilities include chart reviews, plan of care oversight, OASIS familiarity, and driving quality improvement initiatives to support patient outcomes and regulatory readiness.

Qualifications

• Active Illinois RN license required.
• Minimum 2 years of home health nursing experience.
• Case management or care coordination experience in a home health setting.
• Strong knowledge of home health regulations and Medicare requirements.

Responsibilities

• Serve as the primary in-office Clinical Team Manager for assigned home health patients and field clinicians.
• Coordinate care across disciplines to support patient needs and effective clinical care delivery.
• Assess, plan, implement, and evaluate individualized plans of care for home health patients.
• Provide clinical oversight, guidance, and support to field clinicians.
• Monitor patient care to ensure quality, safety, continuity, and compliance with applicable regulations.
• Ensure adherence to Medicare Conditions of Participation, Illinois regulations, and agency policies and procedures.
• Collaborate with physicians, patients, families, field clinicians, and the interdisciplinary team to address clinical needs and barriers to care.
• Review clinical documentation to ensure accuracy, completeness, timeliness, and compliance.
• Participate in quality assurance, performance improvement, and ongoing clinical process improvement initiatives.
• Identify opportunities for clinician education, improved documentation, and enhanced patient care.
• Maintain accurate, timely, and compliant clinical documentation and records.
