Job summary

Community Nurse is seeking a Clinical Manager to lead clinical and administrative staff and ensure delivery of high-quality home health services in Massachusetts. The role involves supervising clinical competency, coordinating patient assessments, and managing documentation and care plans.

The candidate will demonstrate leadership, strong interpersonal skills, and the ability to work both independently and with teams, while managing multiple tasks and reporting to senior administration.

Qualifications

• Licensed as a registered nurse, physical therapist, or occupational therapist in Massachusetts.
• Minimum of 3 years of home health experience preferred.
• Demonstrates leadership, ability to motivate, and good interpersonal skills.
• Ability to work independently & in a team environment to accomplish goals.
• Computer experience (Microsoft 365, EMR software).
• Ability to multitask while maintaining calm demeanor.

Responsibilities

• Make regular home supervisory visits to ensure staff clinical competency including patient assessment, documentation, care plan compliance, and clinically appropriate decision making.
• Monitor documentation of clinical records to assess clear documentation of skill to protect reimbursement.
• Conduct and manage weekly interdisciplinary team meetings.
• Conduct timely performance reviews of assigned staff.
• 2 weekends equivalents per year -supervisor coverage.
