Clinical Magnet, a division of ICON Medical Network, is seeking a Home Health Clinical Manager to lead field clinicians, coordinate high-quality patient care, and ensure the team delivers optimal outcomes in Braintree, MA.

This full-time, direct-hire role requires an unrestricted Massachusetts RN license with 2+ years in home health or hospice, supervisory experience, and travel within the branch’s coverage area.

Qualifications

• Unrestricted Massachusetts RN License.
• Minimum 2+ years RN experience in Home Health or Hospice.
• Supervisory or clinical leadership experience preferred.
• Strong OASIS, care coordination, and clinical knowledge.
• Ability to learn systems such as Homecare Homebase and Workday.
• Willingness to travel within branch’s coverage area.

Responsibilities

• Overseeing patient care, clinical staffing, referrals, and assignments.
• Ensuring timely starts of care and seamless care coordination.
• Reviewing plans of care, physician orders, and clinical documentation.
• Conducting OASIS reviews to support accuracy, quality, and compliance.
• Monitoring patient needs and ensuring care plans remain appropriate.
• Coaching clinicians and providing ongoing education and feedback.
• Supporting recruiting, interviewing, onboarding, and staff development.
• Partnering with physicians, patients, families, providers, and vendors.
• Monitoring key clinical and operational performance metrics.
• Providing field support when needed.
