Adaptive Home Health, LLC is seeking a Clinical Liaison to serve as the on-site bridge between facilities and the field care team. You will accelerate referrals, build partnerships with discharge planners and case managers, and ensure smooth transitions from facility to home health services.

Responsibilities include bedside visits, documentation management, and daily follow-up on referrals to drive admissions, with a compensation model featuring base pay and performance bonuses.

Competenze

• Active, unrestricted Texas license as LVN/LPN, PTA, or COTA (must have active license)
• Minimum 1 year clinical experience in home health, hospital, SNF, or rehab setting
• Strong interpersonal and communication skills for relationship-building with case managers, discharge planners, physicians, and families
• Proficiency with EMR systems and comfortable with data entry and referrals tracking
• Organized and detail-oriented with ability to manage multiple pending referrals
• Reliable transportation and ability to travel between facilities as needed
• Professional appearance and demeanor appropriate for clinical facility environment

Mansioni

• Serve as daily on-site presence at the assigned facility and build relationships with care teams
• Identify patients suitable for home health through proactive engagement with discharge planning
• Conduct bedside visits to introduce Adaptive Home Health and explain services
• Ensure referral paperwork is complete and submitted in real time to intake
• Communicate with intake to expedite referrals and resolve insurance verification issues
• Track pending referrals and follow up until admission and first visit scheduled
• Maintain consistent facility presence and be a trusted partner to staff
