Job summary

Ascension at Home, together with Compassus, seeks a Home Health Clinical Supervisor to model our values and lead clinical operations. You will coordinate all clinical activities within your team and ensure continuity, appropriateness, and high quality of service.

You will supervise and direct clinical personnel, perform patient care assignments, assess and plan care, participate in QA/PI activities, monitor regulatory and payer compliance, and mentor staff to drive positive results.

Qualifications

• Registered nurse with an Associate or Bachelor’s degree in Nursing.
• Minimum of two years’ RN experience and one year in home care.
• Prior supervisory experience preferred.

Responsibilities

• Supervises and leads clinical personnel.
• Performs patient care assignments and staff management.
• Assesses, plans, implements, and supervises patient care.
• Participates in QA/PI activities to improve care quality.
• Monitors cases for regulatory and payer compliance.
• Mentors staff and contributes to performance evaluations.
• Performs other duties as assigned.
