Job summary

Health Wealth Safe, Inc. seeks a Registered Nurse Home Health Case Manager to provide personalized patient care in clients' homes.

You will coordinate services among healthcare providers and oversee visiting nurses while developing individualized care plans based on assessments and physician orders. You will manage caseloads, supervise LPN visits, and conduct routine and start-of-care visits, delivering education on CHF, COPD, Diabetes, wound care, and medication management.

Qualifications

• Must possess Kansas or Compact RN license.
• Valid Driver's License required.
• Reliable transportation required.

Responsibilities

• Coordinate services among healthcare providers, communicate with insurance companies, and oversee visiting nurses.
• Develop individualized care plans based on assessments, physician orders, and diagnosis-driven strategies.
• Manage caseloads, supervise LPN visits, and perform routine, evaluation, and start-of-care visits.
• Provide education on health conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management.
• Collaborate with multidisciplinary teams including Physical, Speech, and Occupational Therapists.
