Job summary

Phoenix Home Care and Hospice is seeking a dedicated RN for our Springfield, MO service area to provide patient-centered home health care. You will assess patients, develop and revise plans of care, and coordinate with physicians and therapists to ensure quality outcomes.

The role emphasizes strong nursing assessment, communication, and the ability to work with a multidisciplinary team in a home setting, with on-call weekend shifts as part of the schedule.

Qualifications

• Current Registered Nurse license in Missouri or Compact required.
• Home Health or Critical Care experience preferred.
• Driver's license and vehicle with current auto insurance.
• Ability to pass drug test, background check, and physical.

Responsibilities

• Assists in development, review and revision of the plan of care.
• Provides nursing care in accordance with physician's plan of treatment and individual plan of care.
• Observes, records and reports reactions to treatment and changes in patient condition to appropriate personnel and/or physician.
• Provides ongoing instruction and supervision of patient, family or other health team personnel in the plan of treatment.
• Participates in education programs, quality assessment activities, nurse meetings and other staff meetings as required.
• Monitors and records patient's condition and documents provided care services via electronic charting.
