Job summary

Energy Employee Services, LLC, in Kansas City, KS, is seeking a compassionate Registered Nurse Case Manager to join our home health team. You will manage the home care of terminally ill or elderly patients with strong communication and dedication to accessible, comprehensive care.

Responsibilities include home assessments, overseeing the Plan of Care, guiding field staff, coordinating with providers, educating on medications, and documenting status. This is a full-time role with on-call rotation.

Qualifications

• Must have an unencumbered RN license in the state of application.
• Must hold an active CPR certification and a valid driver's license.

• Oversee the patient’s Plan of Care (POC) as provided by the physician.
• Guide and mentor Field Staff to ensure Plan of Care is followed accurately.
• Work closely with staff and patient providers to ensure best outcomes.
• Provide clear medication education and medication management.
• Assist with activities for daily living to improve quality of life.
• Participate in on-call rotation as assigned.
• Timely and accurate documentation of patient status and condition.
• Other tasks as assigned.

Position Overview

We are seeking a compassionate Registered Nurse Case Manager to join our team. As a member of the home health team, you will be responsible for managing the home care of terminally ill or elderly patients. The ideal candidate will also have strong communication skills and a committed dedication to providing patients and families with accessible, comprehensive care.

Responsibilities

• Thoroughly assess patients in their homes to ensure best patient care.
• Oversee the patients Plan of Care (POC) as provided by the physician
• Guide and mentor Field Staff to ensure Plan of Care is being followed accurately
• Work closely with staff and patients provider to ensure best outcomes for the patient.
• Provide clear medication education and medication management
• Assist with Activities for Daily Living (ADLS) to help with the patients quality of life.
• Must be willing to participate in the On-call rotation as assigned, to ensure best patient care and outcomes!
• Timely and accurate documentation of patient status and condition.
• Other tasks as assigned.

Qualifications

• Must be willing to submit to a background check
• Must have a valid driver license, reliable transportation and auto insurance
• Must have an unencumbered RN License in the state you are applying to.
• Must be willing to travel to patient’s home (This can be up to a 45 mile radius)
• Must hold an active CPR Certification
