Responsibilities

• Assist with IADLs (grocery shopping, cleaning, meal prep, etc.).
• Document services in EHR accurately and timely.
• Participate in care planning with multidisciplinary teams (IDT).
• Follow universal precautions and biohazard safety procedures.
• Attend required trainings and staff meetings.
• Other duties as assigned.

Assists in monitoring the health status and providing a safe environment for participants. Observes participants taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely manner* Assists participants with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assigned* Documents services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely manner* Participates in participants care planning through cooperation with multidisciplinary teams* Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions* Participates in Company required and assigned training programs and any required staff and training meetings* Other duties as requested/assigned## Qualifications* High school diploma or equivalent* Minimum of one year of experience with a frail or elderly population preferred* Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers* Must have a current Colorado Drivers License and driving record must be in compliance with Company policy.
