Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will collaborate with an interdisciplinary team, coordinate with families and referring agencies, and ensure care quality and continuity.

The successful candidate will have a nursing degree, an active RN license, and at least 1–2 years in a psychiatric or home health setting, with CPR on file and reliable transportation.

Qualifications

• Graduate from accredited nursing program (Diploma/ASN/BSN).
• 1-2 years nursing experience in psychiatric/behavioral health preferred.
• Current RN license in the state.
• CPR certification and hepatitis consent/declination.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Perform in-home psychiatric evaluations and nursing interventions.
• Coordinate care with interdisciplinary team, patient, family, and referrals.
• Maintain accurate patient medication profiles and regulatory documentation.
• Complete admissions, recerts, and discharges per standards.
• Demonstrate proficiency with EMR systems and OASIS where applicable.
