Jefferson Healthcare is seeking a full-time Home Health and Hospice Aide (CNA) to deliver compassionate in-home care under the guidance of an RN Case Manager.

You will follow a prescribed plan of care, assist patients with daily activities, and promote comfort and dignity while ensuring safety and accuracy of documentation. Washington NA-C and CPR certifications are required.

Qualifications

• High School diploma or GED required.
• Current Washington State Nurse Assistant Certified (NA-C) required.
• AHA BLS/CPR (Healthcare Provider) required.

Responsibilities

• Provide direct personal care to patients in their homes following a plan of care prepared by the RN Case Manager.
• Support patients with activities of daily living while promoting comfort and dignity.
• Document care provided and report changes to supervising RN.
