Job summary

Concordia Visiting Nurses, a Medicare-certified home health agency serving nine Western Pennsylvania counties, seeks a compassionate Home Health Aide (HHA) to provide person-centered in-home care. You’ll support daily activities, offer companionship, assist with mobility and transfers, and help monitor vital signs as part of a respectful care team.

Home Health Aide Position

What you’ll do as a Home Health Aide HHA

• Assist clients with daily activities, including bathing, dressing, grooming, and personal hygiene.
• Provide companionship, emotional support, and engage in activities to promote well-being.
• Assist with mobility and transfers, including the safe use of assistive devices like wheelchairs and walkers.
• Monitor and record vital signs, reporting any significant changes to the healthcare team.
