Concordia Visiting Nurses Cabot is seeking a Home Health Aide (HHA) to provide in‑home support and personal care for patients in the Cabot, PA area. You will assist with daily activities, monitor health status, and deliver compassionate, patient‑centered care in clients' homes.

Qualifications

What you’ll do as a Home Health Aide HHA:

• Assist with mobility and transfers, including the safe use of assistive devices like wheelchairs and walkers.
• Monitor and record vital signs, reporting any significant changes to the healthcare team.
