Job summary

Enhabit Home Health & Hospice is seeking a dedicated caregiver to provide patient care as ordered in the plan of care. You will support licensed clinicians by assisting with daily activities and documenting care using mobile devices.

A CNA or HHA certificate is required, with state registration in good standing. Requirements include a valid driver license, reliable transportation, CPR certification, and ability to safely drive in all weather.

Qualifications

• One year of full-time direct client care in an institutional setting (hospital or nursing facility) or one year in home health/hospice care within the last five years.
• Must be registered and listed in good standing on the nurse aide registry in the state of employment.
• Basic technology skills, including operation of a mobile device.

Responsibilities

To provide patient care related services and tasks as ordered by the physician in the plan of care.

Qualifications

• Must meet one of the following requirements: 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); or 2) have one year experience within the last five years in direct client care in a home health or hospice setting.


• Must be registered and listed in good standing on the nurse aide registry in the state of employment.


• Must have basic demonstrated technology skills, including operation of a mobile device.
