Job summary

The University of Texas Southwestern Medical Center is seeking a Home Health Aide (PRN) to provide fundamental home nursing services under nursing direction in Dallas, TX. The role involves assisting with ADLs, vital signs, and maintaining patient records with strict confidentiality.

Eligible candidates typically have a high school diploma, CNA certification, BLS, and valid Texas driver's license. Prior direct patient care experience in hospital, nursing facility, or agency settings is preferred.

Qualifications

• High School Diploma or equivalent required.
• CNA certification listed on NAR preferred.
• 1 year of full-time direct client care in an institutional setting.
• 1 year of direct client care in an agency setting within the last 5 years.

Responsibilities

• Provide basic home nursing services as directed by a nurse.
• Record patient vital signs and document observations accurately.
• Report changes in patient condition to nursing supervisor.
• Assist with ADLs, light housekeeping, meal prep, and mobility tasks.
• Maintain patient records while ensuring confidentiality.
• Attend mandatory in-service educational programs.

JOB SUMMARY

Under direction of registered nurse, licensed physical therapist, or a licensed occupational therapist provides basic home nursing services to patients.

  1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility) and
  1 year of full-time experience within the last five (5) years in direct client care in an agency setting.

JOB DUTIES

• Performs duties as assigned on the home health care plan prepared by a registered nurse.
• Obtains and documents patient vital signs, including temperature, weight, respiration, and pulse.
• Reports changes in patient's mental or physical condition or in home situation to nursing supervisor.
• Assists other members of the nursing team and/or other disciplines.
• Provides personal care assistance to patients with all Activities of Daily Living (ADLs), light housekeeping, meal preparation, and other specialized duties as needed.
• Provides assistance to patients with ambulation, safe transfers, and exercises as assigned.
• Maintains patient records and charts to ensure an accurate record of treatment.
• Maintains confidentiality of patient data.
• Assists in maintaining a healthy, safe environment.
• Attends mandatory in-service educational programs as required by federal and state regulations.
• Performs other related duties as assigned or requested.

Knowledge, Skills and Abilities

• Work requires ability to care for and treat patients in accordance with established protocols, accepted nursing standards, and instructions from physicians and/or nurses.
• Work requires verbal and written communication skills.
• Work requires interpersonal skills.
• Work requires contact with physicians, residents, fellows, secretaries, and allied health personnel to coordinate information necessary to provide optimal level of patient care.
• Work requires ability to stand for long periods of time.
• Work requires ability to lift, push, and/or pull up to 50 lbs.
• Work requires ability to read, write, and perform mathematical calculations.
• Work requires ability to perform under general supervision.
• Work requires that incumbent complete most assignments with substantial independence
