Cypress Garden Center for Nursing & Rehabilitation is seeking a compassionate and dependable Home Health Aide (HHA) to join our team on a PRN basis. You will assist residents with transportation to medical appointments, support safety during outings, and provide a caring presence throughout the day.

This role requires current New York HHA certification, excellent communication, and the ability to work independently while maintaining professional standards and quality resident care.

Qualifications

Responsibilities

• Escort residents to and from medical appointments and off-site visits.
• Ensure safety during transportation and monitor fall risks.

• Report incidents or changes in resident condition promptly to the Clinical Coordinator or ADNS.
• Maintain professionalism while off-site and adhere to facility policies.

Job Summary

Cypress Garden Center for Nursing & Rehabilitation is seeking a compassionate and dependable Home Health Aide (HHA) to join our team on a PRN basis. You will provide essential support to residents by assisting with transportation to medical appointments, ensuring safety during outings, and offering a caring presence throughout the day.

Key Responsibilities

• Escort residents to and from clinic appointments, dialysis treatments, and other off-site medical visits.

• Ensure resident safety during transportation and appointments, monitoring for fall risks or other concerns.

• Assist residents with preparation for appointments, including dressing and gathering necessary items.

• Provide snacks and support during out-on-pass periods as needed.

• Promptly report incidents, accidents, or changes in resident condition to the Clinical Coordinator or ADNS.

• Maintain professionalism and represent the facility positively while off-site.

• Adhere to all facility policies and procedures at all times.

Minimum Qualifications

• Current New York Home Health Aide (HHA) certification required.

• Strong communication and interpersonal skills.
