Job summary

VitalCaring, a nationwide home health and hospice provider, seeks a compassionate Home Health Aide (HHA) to support patients in their homes across Oklahoma. You will assist with daily living activities, monitor health, and document care in the EMR, collaborating with nurses and care teams to ensure safe, respectful care.

We value empathy, reliability, and strong time management. A valid HHA certification or training, CPR, and a driver’s license are required to join our team.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• Age 18 or older with ability to read and follow care plans.
• CPR certification and a valid driver’s license required.

Responsibilities

• Assist patients with daily living activities such as bathing, dressing and grooming.
• Support mobility, transfers, and basic exercises according to the care plan.
• Maintain a clean, safe home environment and assist with meal preparation.
• Take and record vital signs as trained and required.
• Document care provided accurately in the EMR and report changes to the clinical team.
• Participate in team meetings and care coordination activities.
• Provide compassionate support to patients and families during visits.

What You'll Do

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self‑administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

What We're Looking For

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skills
• Demonstrates empathy, patience, and professionalism in all interactions
