Community Home Health Care Inc. is hiring Home Health Aides to support older adults and individuals with developmental possibilities in Fort Wayne, Indiana.

We offer full-time and part-time schedules, weekend shifts with premium pay, plus early morning and evening hours and holiday shifts to boost income. Responsibilities include assisting with personal care, mobility, meal prep, companionship, and safe documentation of visits.

Qualifications

Responsibilities

• Assist with personal care and daily routines while preserving client dignity.
• Support safe mobility, walking, and transfers as directed.
• Help with meal preparation, feeding, and light housekeeping when included.
• Provide companionship and emotional support.
• Follow care plans, agency procedures, and client preferences.
• Report changes in condition promptly and maintain documentation.
• Communicate clearly with clients, families, and the care team.
• Complete visit documentation accurately and on time.
• Respect client privacy and maintain professional boundaries.
