Community Home Health Care & CIHC is hiring Home Health Aides to support older adults and individuals with developmental possibilities in Fort Wayne, Indiana. We offer full-time and part-time opportunities, with weekend shifts and premium pay, plus early morning, evening, and holiday hours to boost income.

We seek compassionate, patient, dependable caregivers who can follow care plans, maintain client dignity, assist with personal care, meals, mobility, and light housekeeping, and report changes

Responsibilities

• Assisting with personal care and daily routines while protecting each client's dignity
• Supporting safe mobility, walking, and transfers as directed

• Offering companionship and respectful emotional support
• Following care plans, agency procedures, and client preferences
• Noticing and promptly reporting changes in a client's condition or needs
• Communicating clearly with clients, families, and the care team
• Completing visit documentation accurately and on time
• Respecting client privacy and maintaining professional boundaries
