Job summary

A Place at Home is seeking a dedicated Home Health Aide to support clients in their own homes with daily living activities and compassionate care. You will work under the supervision of registered nurses to ensure high-quality, individualized care plans are followed.

Responsibilities include assisting with ADLs, meal planning, medication reminders, mobility support, and documenting changes for the supervising nurse. A strong, empathetic communicator with a valid HHA certification is essential.

Qualifications

• Completion of state-approved Home Health Aide certification course.
• Strong communication skills, both written and verbal.

• Ability to work independently and maintain high levels of patience and understanding.
• Physically able to perform the duties of the position, including lifting and physical movement as necessary.
• Reliable transportation and valid driver’s license.

• Following HIPAA guidelines and maintaining the privacy of all client PHI (Personal Health Information).
