Home Care Evolution is seeking a dedicated Home Health Aide to provide one-on-one care in patient homes in Gainesville, FL. You will follow individualized care plans, document patient progress, and work closely with supervisors to ensure safety and well-being.

Qualifications

• Graduated from an accredited Home Health Aide program OR able to pass the Florida Home Health Aide competency test that we will administer prior to your interview.
• High School Diploma or GED.
• More than five years prior experience in senior caregiving.
• Valid driver’s license.
• Proof of auto insurance with your name on the policy.
• Proof of current CPR certification (American Heart Association compliant).
• Ability to pass AHCA background screening

Responsibilities

• Assist patients with daily activities such as transferring in and out of beds, baths, wheelchairs, or automobiles
• Care for clients by changing bed linens, doing laundry, cleaning the home, preparing meals, or assisting with personal care
• Maintain records of client care, condition, progress, or problems to report and discuss observations with supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medication reminders
• Engage patients in exercises or other activities

We are looking for a Home Health Aid to join our team! You will directly work with clients, following a one-on-one care plan in patient homes.

You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.

Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
