Job summary

One Home Health Agency, Ltd. is seeking compassionate Home Health Aides (HHA) and CNAs for part-time and PRN shifts in the Chicago area (Downtown Chicago and North Shore suburbs).

The role focuses on one-on-one patient care in patients' homes with flexible scheduling to fit your lifestyle. Responsibilities include ADL assistance, mobility support, caregiving within care plans, and collaboration with the clinical team.

Qualifications

• Active Illinois CNA certification (or qualified Home Health Aide credential)
• Previous home health, hospice, private duty, or caregiving experience preferred
• Reliable, patient-focused approach to care
• Valid driver's license and reliable transportation preferred

Responsibilities

• Provide personalized care in patients' homes
• Assist with activities of daily living: bathing, dressing, grooming, mobility
• Observe and report changes in patients' condition to the clinical team
• Assist with medication reminders and approved personal care services
• Follow individualized care plans and agency protocols
• Educate and support family members and caregivers as appropriate
• Maintain a safe, clean, and respectful care environment
• Protect patient privacy and confidentiality at all times
