Triniti Home Health & Hospice is seeking a dedicated Home Health Aide (CNA) to provide compassionate in-home care to patients in the West Metro area. This full-time role supports individuals with chronic illnesses, hospice needs, and daily living activities, while ensuring dignity and comfort.

Qualifications include Minnesota CNA certification and Medicare competency; prior home care or hospice experience is preferred.

• Monitor patient conditions and report changes to RN Case Manager.
• Document visits accurately and on the same day.
• Participate in in-services, education, and team meetings.
• Rotate weekends as required.
• Maintain HIPAA compliance and patient confidentiality.

As a Home Health Aide (CNA), you will provide compassionate, hands‑on personal care to patients with chronic illnesses, disabilities, or those receiving hospice care while helping them maintain their dignity, comfort, and quality of life. Our ideal candidate is dependable, compassionate, and committed to delivering exceptional care while building meaningful relationships with patients and their families. This position does both home health and hospice visits.

Position Type: Full-Time, benefit-eligible position based in the West Metro area. Wage Range: $19 - $25 / hour depending on experience.

Home Health Aide (CNA) Responsibilities:

• Provide personal care services including bathing, dressing, grooming, toileting, oral care, positioning, transfers, and bed changes.
• Assist with mobility, therapeutic exercises, and active or passive range of motion as directed.
• Support patients with meal preparation, feeding, light housekeeping, laundry, and maintaining a clean, safe home environment.
• Monitor patient conditions and promptly report changes to the RN Case Manager or RN Supervisor.
• Perform delegated nursing or therapy procedures as directed by the supervising RN or therapist.
• Provide emotional support and companionship while promoting patients’ comfort, dignity, and mental well‑being.
• Complete accurate and timely electronic documentation, with at least 90% completed at the point of care and all visits documented the same day.
• Participate in mandatory in‑services, complete annual education requirements, and attend team meetings as required.
• Participate in rotating weekends based on patient care needs.
• Maintain compliance with all state and federal home health and hospice regulations, including HIPAA requirements.
• Perform other duties as assigned.

Home Health Aide (CNA) Qualifications:

• Current Minnesota Certified Nursing Assistant (CNA) certification.
• Ability to successfully complete Medicare competency requirements.
• Previous experience in home care, hospice, long‑term care, or a related healthcare setting is preferred.
• Strong communication, interpersonal, and customer service skills.
• Compassionate, patient‑focused approach with the ability to support individuals with chronic illnesses, disabilities, or those receiving end‑of‑life care.
• Ability to work independently while collaborating effectively with the interdisciplinary care team.
• Strong organizational skills and attention to detail.
• Ability to recognize emergencies and respond appropriately.
• Commitment to maintaining patient confidentiality and compliance with HIPAA guidelines.
• Reliable transportation and the ability to travel to patient homes throughout the service area.
