Job summary

TheraCare Home Health in Dallas, TX is seeking a Home Health Aide to provide personal healthcare in clients' homes under RN supervision, promoting safe, clean environments and coordinating with families.

The role emphasizes compassionate patient care, accurate documentation, infection control, and collaboration with the clinical team to support discharge planning and quality of care. Valid HHA certification, experience, and CPR/Hepatitis profile are required.

Qualifications

• Completion of Certified Home Health Aide Program.
• 1 year of Home Health Aide experience in a home health agency.
• Reliable transportation. Valid and current auto liability insurance.
• Good communication skills to relate professionally to patients, caregivers, and clinical team.
• Ability to read, write, and speak English.
• Demonstrates interest in the welfare of ill and elderly.
• Proof of current CPR and Hepatitis profile.
• Aide Competency Evaluation Skills Checklist and Written Skills Test.

Responsibilities

• Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy.
• Report changes and document pertinent information to ensure continuity of care.
• Practice infection control principles and maintain a clean, safe environment.
• Promote respectful communication to patient/family and other employees.
• Respect patient privacy and property.
• Assist with proper nutrition and adequate fluid intake.
• Support efficient operation of the agency with good time management.
• Perform treatments and procedures according to agency policy.
• Attend 12 hours of in-services per year.
• Promote agency philosophy and policies to ensure quality of care.
• Record clinical documentation in medical records.
• Participate in weekly case conferences.
• Engage in interdisciplinary communication to align patient needs.
• Ensure patient safety.

Job Summary:

The Home Health Aide provides personal healthcare and related services to the client in their place of residence, assists in providing a safe and clean environment, works cooperatively with patient and family, & shares observations and problems with the supervisor. The HHA works under the direction and supervision of a registered nurse. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.

Qualifications:

• Demonstrate commitment, professional growth and competency by attending 12 hours of in-services per year.
• Promote the agency philosophy and administrative policies to ensure quality of care.

• Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
