Job summary

Holy Redeemer Home Care and Hospice is seeking dedicated nursing support to provide personal care to clients in home and hospice settings in the Philadelphia area. Under supervision of the Case Manager and Clinical Manager, you will assist with daily living activities and monitor patient condition.

A high school diploma is preferred; good communication, ability to follow instructions, basic computer skills for entry of patient data, and valid driver's license with access to a car are required.

Qualifications

• High School diploma or equivalent preferred.
• Able to read and understand written instructions.
• Able to take direction via telephone in patient home.
• Capable of observing and reporting changes in patient condition.
• Basic computer skills to enter patient information into an electronic device.

SUMMARY OF JOB

Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager.

RECRUITMENT REQUIREMENTS

High School diploma or equivalent preferred;Must be able to read and understand written instructions; Must be able to take direction via telephone in patient’s home; Must be able to make meaningful observations and report changes in patient condition; Must have basic knowledge of computer applications and ability to enter patient information into an electronic device. Successfully pass all competency exams.

LICENSE AND REGULATORY REQUIREMENTS

Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver’s license and access to a car, current car registration, and current car insurance.
