Job summary

Buckeye State Health and Family Services is seeking a dedicated Home Health Aide to provide compassionate in-home care, assisting patients with daily living activities and promoting independence and comfort in their own homes.

You will support seniors, individuals with disabilities, or those recovering from illness, perform light housekeeping, document progress, and communicate with healthcare teams to ensure safe, quality care.

Qualifications

• Experience in home health care or long-term care environments.
• CPR certification is required.
• Knowledge of medical terminology for clear observations and communications.
• Experience documenting patient progress and care plans.
• Ability to assist with ADLs and use basic assistive devices.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
• Support patients with specialized care needs (e.g., Alzheimer’s, dementia, hospice, catheter care, developmental disabilities).
• Perform light housekeeping: cleaning, laundry, meal prep, and safe living environment maintenance.
• Document patient progress and communicate changes to healthcare teams.
• Use medical terminology when recording observations and communicating with professionals.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.

• Support patients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.

• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.

• Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.

• Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.

Skills

• Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices.

• Strong patient observation skills to detect changes in condition promptly and accurately.

• Experience working with individuals with disabilities or in assisted living settings is highly valued.

• Knowledge of medical terminology along with CPR certification to respond effectively in emergencies.
