Job summary

Cantex Continuing Care Network is seeking a reliable Home Health Aide (HHA) to provide high-quality personal care to patients in the home setting in Beaumont, TX.

You will support daily living activities, promote a safe environment, and work with the clinical team to ensure continuity of care.

Qualifications

  • Completion of a Certified Home Health Aide (HHA) Program.
  • Minimum of 1 year of experience as an HHA in a home health agency.
  • Ability to follow an established Plan of Care under RN supervision.
  • Excellent communication with patients, families, and clinical staff.
  • Demonstrated compassion and commitment to the care of elderly and ill patients.
  • Ability to accurately document care and report changes in patient condition.
  • Reliable transportation with valid driver’s license and current auto liability insurance.
  • Current CPR certification and required health screenings.
  • Ability to read, write, and communicate effectively in English.

Responsibilities

  • Provide direct patient care under RN supervision.
  • Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting.
  • Support safe patient transfers, ambulation, and positioning.
  • Monitor and report changes in patient condition to the supervising nurse.
  • Maintain a clean, safe, and comfortable home environment for patients.
  • Assist with proper nutrition and hydration based on patient needs.
  • Follow and maintain infection control and safety protocols.
  • Complete accurate and timely documentation in the medical record.
  • Communicate effectively with patients, families, and interdisciplinary team members.
  • Participate in interdisciplinary communication and case conferences as needed.
  • Maintain patient dignity, privacy, and confidentiality at all times.
  • Support efficient operations through strong time management and reliability.
  • Attend required in-services to maintain competency and compliance.
  • Promote agency policies, procedures, and commitment to quality care.
