Job summary

Cantex Continuing Care Network is seeking an RN Case Manager in Beaumont, TX to supervise patient care, coordinate visits and discharge planning, and collaborate with the clinical team to meet patients' medical and social needs.

The role requires an active RN license and Home Health experience is preferred. Strong communication and documentation skills, plus reliable transportation and a valid auto policy, are essential.

Qualifications

  • HomeCare HomeBase experience preferred.
  • Good communication, assessment, and clinical documentation skills.
  • Ability to read, write, and speak English.
  • Reliable transportation with auto liability insurance.

Responsibilities

  • Supervises and manages all aspects of patient care, including LVN and HHA supervision.
  • Coordinates visits across disciplines after Start of Care/Recert/Resumption; coordinates POC with physician.
  • Initiates referrals to ancillary services (podiatry, community services).
  • Facilitates interdisciplinary communication to optimize service delivery and patient progress.
  • Leads weekly interdisciplinary case conferences and contributes to care plans.
  • Establishes social action plans and goals aligned with patients' needs.
  • Interprets and promotes Patients' Rights with Administrator support.
  • Coordinates discharge planning with patients, clinical team and agency.
  • Provides individual and group support for patients/residents and families during adjustment or crisis.
  • Completes necessary documentation per state and federal regulations.
  • Acts as a team leader/role model; serves on committees as requested by Administrator.
  • Familiar with Abuse Prevention policies and enforces responsibilities.
  • Responsible for patient safety.
  • Performs other duties as assigned.

Location of Service Area:Beaumont, TXJob Summary:The overall purpose of the RN Case Manager position is to conduct all aspects of supervision and management of patient care, including LVN and HHA supervision, case conferences and discharge planning to enable the agency to identify the medically related social and emotional needs of the patients. This position will promote patients’ potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.Qualifications:Registered Nurse, licensed per the state requirements.2 years of experience in Home Health preferred.HomeCare HomeBase experience preferred.Good communication skills, assessment skills, and strong clinical documentation skills.Ability to read, write, and speak the English language.Reliable transportation. Valid and current auto liability insurance.Case Manager Essential Functions:Supervises and manages all aspects of patient care, including LVN and HHA supervisionCoordinates the distribution of visits among all disciplines (SN, PT, OT, ST, MSW, HHA) after the Start of Care, Recertification or Resumption of Care is completed, and coordinates POC with the physician.
