Job summary

VitalCaring seeks compassionate Home Health Aide (HHA) to provide hands-on care in patients' homes in Beaumont, TX. You will support daily living activities, help with mobility and safety, and document care in the EMR system.

Qualifications

Responsibilities

• Provide hands-on home care focusing on comfort, safety, and independence.
• Assist with activities of daily living and basic exercises per care plan.
• Maintain a safe, clean home environment and assist with meals.
• Record vital signs and medications self-administration as directed.
• Document care in the EMR system accurately and timely.
• Participate in team meetings and care coordination activities.

What You’ll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

• Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
• Support mobility, transfers, and basic exercises in alignment with the care plan
• Help maintain a clean, safe, and comfortable home environment
• Prepare meals and assist with feeding as needed
• Take and record vital signs when required and trained to do so
• Assist with medication self-administration as directed
• Observe and report changes in patient condition to the clinical team
• Document care provided accurately and timely within the EMR system
• Participate in team meetings, trainings, and care coordination activities
• Provide compassionate support to patients and families during each visit

What We’re Looking For

We’re seeking individuals who are dependable, compassionate, and comfortable working independently in a home-based setting:

• Meets state and federal requirements for Home Health Aide certification or training
• At least 18 years of age
• Ability to read, follow care plans, and document services provided
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to work independently with strong time management skillsDemonstrates empathy, patience, and professionalism in all interactions
