Job summary

TheraCare Home Health is seeking a reliable and compassionate Home Health Aide (HHA) to provide high-quality personal care to patients in the home setting. You will support patients with daily living activities, promote a safe and comfortable environment, and work closely with the clinical team to ensure continuity of care.

This role offers flexible PRN scheduling and the opportunity to make a meaningful impact in home health.

Qualifications

• Completion of a Certified Home Health Aide (HHA) Program.
• 1 year of experience as a Home Health Aide preferred in home health agency.
• Ability to follow Plan of Care under RN supervision.
• Strong communication with patients, families and staff.

Responsibilities

• Provide direct patient care under RN supervision.
• Assist with activities of daily living (bathing, grooming, dressing, toileting).
• Support transfers, ambulation, and positioning.
• Monitor and report changes in patient condition.
• Maintain clean, safe home environment.
• Support nutrition and hydration based on patient needs.
• Follow infection control and safety protocols.
• Complete accurate documentation in medical record.

Job Summary:

TheraCare Home Health is seeking a reliable and compassionate Home Health Aide (HHA) to provide high-quality personal care to patients in the home setting. In this role, you will support patients with daily living activities, promote a safe and comfortable environment, and work closely with the clinical team to ensure continuity of care. QualificationsQualifications:

• Completion of a Certified Home Health Aide (HHA) Program
• Minimum of 1 year of experience as a Home Health Aide in a home health agency
• Strong understanding of personal care, patient safety, and infection control practices
• Ability to follow an established Plan of Care under RN supervision
• Excellent communication skills with patients, families, and clinical staff
• Demonstrated compassion and commitment to the care of elderly and ill patients
• Ability to accurately document care and report changes in patient condition
• Reliable transportation with valid driver’s license and current auto liability insurance
• Current CPR certification and required health screenings
• Ability to read, write, and communicate effectively in English

Essential Functions:

• Provide direct patient care under the supervision of a Registered Nurse in accordance with the Aide Plan of Care
• Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing and toileting
• Support safe patient transfers, ambulation, and positioning
• Monitor and report changes in patient condition to the supervising nurse
• Maintain a clean, safe, and comfortable home environment for patients
• Assist with proper nutrition and hydration based on patient needs
• Follow and maintain infection control and safety protocols
• Complete accurate and timely documentation in the medical record
• Communicate effectively with patients, families, and interdisciplinary team members
• Participate in interdisciplinary communication and case conferences as needed
• Maintain patient dignity, privacy, and confidentiality at all times
• Support efficient operations through strong time management and reliability
• Attend required in-services to maintain competency and compliance
• Promote agency policies, procedures, and commitment to quality care
