Job summary

Home Care Evolution is seeking a Certified Home Health Aide (HHA) to provide one-to-one patient care in Lakewood, CO. The role emphasizes compassionate ADL support, medication monitoring, infection control, and clear documentation, with flexible shifts and mileage reimbursement.

You will follow care plans, engage clients therapeutically, and collaborate with supervising nurses to ensure safety and quality care in the home setting.

Qualifications

• Completion of a state-approved Home Health Aide program (HHA certificate).
• Minimum 1 year of paid home-health or hospice experience.
• Valid driver’s license and reliable transportation to reach clients’ homes.
• CPR certification (or willingness to obtain within 30 days).
• Clear, confident communication and a passion for relationship-centered care.

Responsibilities

• Hands-On ADL Support – transfers, bathing, grooming, toileting, and mobility.
• Home Environment & Infection Control – change linens, light laundry, and maintain a hazard-free, sanitized space.
• Medication & Symptom Monitoring – provide reminders, observe changes, and document vitals or red-flag signs for your supervising nurse.
• Therapeutic Engagement – read aloud, reminisce, guide ROM exercises, or accompany short walks to boost wellness.
• Accurate Documentation – record care notes, progress, and concerns in our digital charting system before each shift ends.
